Appointments

If you are an existing patient seeking to transition to Horizon Neurology, or a new patient in need of a movement or headache disorders neurologist, we are beginning the process of pre-registration.

Because we are still in the process of insurance credentialing, we are starting in a limited manner. At present, we are only able to pre-register the following:

  • Patients with regular Medicare part B (commercial supplements are fine)

  • Patients with select commercial/employer Anthem plans (PPO/HMO/POS)

  • Patients with other commercial insurance who are interested in seeing us out-of-network (self-pay)

Due to credentialing status and rigid state/federal regulations, we are not presently able to pre-register patients with any form of Medicare Advantage or Medicaid coverage.

Special note on Botox injections

If you are an existing patient of Dr. Julius and wish to continue Botox injections with him at Horizon Neurology, there are a few critical steps that we must navigate:

  • A full 3 months must elapse in between each Botox injection - otherwise, insurance will not cover it - when you schedule your injection, make sure it is 3 months after the last one

  • Regular Medicare part B (when primary) does not require pre-authorization for Botox - if this is your primary insurance, you can schedule your injection with no further hassle

  • If you have commercial insurance - we must get pre-authorization for Botox. Because the authorization is tied to the physician and the location, this requires several steps:

    • Schedule two visits at Horizon Neurology:

      • An initial follow-up visit (office or telehealth) - the earlier in January, the better - we will need this to obtain new Botox authorization

      • A second injection visit 1-2 weeks later - make sure the date is at least 3 months after your previous Botox injection

    • Contact your previous practice - tell them to cancel your current Botox authorization (we are not allowed to do this on your behalf) - if one Botox authorization is still active, insurance will not allow us to start a new one

    • Once the above steps are done, Horizon Neurology can then obtain the new authorization to keep you on track with your injections
